New Client Information
Please fill out the form below after the appointment is made.
Hours are by appointment only. To schedule an appointment, please fill out the Request Appointment Form first and then complete the New Client Form. This will reduce the time it takes to check you in when you arrive for your appointment. If you do not keep a copy of your pet's medical records, please call the veterinary office that holds your records and request a copy of your records, to have at the time of your appointment.
Some flexibility in our time of arrival for scheduled appointments is necessary. We will give you a half hour window for our arrival, but may be slightly early or late depending on the location and nature of appointments prior to yours. We will call if our arrival time varies from your assigned appointment window. You should generally call several days in advance to schedule appointments. If you need to reschedule or cancel, 24 hours notice is required to avoid forfeiture of your pre-paid appointment confirmation.
Please have your pet on a leash and or in a carrier prior to your appointment time. All appointments, examinations, treatments and procedures will take place inside the mobile clinic unless prior arrangements have been made.
